Indicated / Suitable For
- Adults with a confirmed cancer diagnosis and an available pathology report who want a second opinion on treatment options, including targeted therapy or immunotherapy.
- Patients with advanced or recurrent cancer for whom tumour molecular profiling, such as next-generation sequencing (NGS), may help identify treatment options, as judged by the oncologist.
- Patients with stored tumour tissue (paraffin blocks or slides), or who can have a new biopsy or a blood-based liquid biopsy if it is clinically appropriate.
- Patients whose general condition allows safe travel and who can attend scheduled consultations, blood tests and imaging in Istanbul.
- Patients whose local oncologist or family doctor can share monitoring and continue care after they return home.
Contraindications / Not Suitable
- Oncological emergencies, such as spinal cord compression, severe bleeding, fever with low blood counts, breathing difficulty, bowel obstruction or sudden confusion, must be treated immediately at a local emergency department.
- Very poor general condition or organ function, where travel and systemic treatment may cause more harm than benefit; the oncologists may then recommend supportive care close to home.
- For immunotherapy, certain active autoimmune diseases, a previous organ transplant or conditions needing high-dose steroids may make checkpoint inhibitors unsuitable or require special caution.
- Pregnancy or breastfeeding, as many targeted drugs and immunotherapies can harm the baby; effective contraception is usually required during treatment.
- No possibility of follow-up at home; side effects of these drugs can become serious if unmonitored, so starting treatment abroad without a local monitoring plan is generally not appropriate.
Pre-Operative Evaluation & Diagnostic Checks
Pathology & Records Review: Original pathology reports and, when requested, tissue blocks or slides for review, together with treatment history, operation notes and discharge summaries, translated for the specialists.
Staging Imaging: Recent CT, MRI or PET-CT scans (images on disc plus reports) to confirm the current stage; repeated in Istanbul if outdated or incomplete.
Molecular & Biomarker Testing: Tumour genomic profiling (NGS), PD-L1, MSI or MMR status and other markers relevant to the cancer type, from tissue or blood, where the oncologist considers it useful.
Baseline Blood Tests & Organ Function: Blood counts, liver and kidney function, thyroid function, blood sugar and hepatitis serology, plus cardiac checks (ECG, echocardiography) when the selected drug requires them.
Tumour Board Review: Multidisciplinary discussion by medical oncology, radiation oncology, surgery, pathology and radiology before any treatment option is proposed.
Potential Risks & Complications
In compliance with transparent clinical reporting, Meva Clinic ensures patients are fully informed of the physiological risks associated with surgical procedures through accredited partner-hospital safety protocols where clinically appropriate.
Immunotherapy can cause inflammation in almost any organ, such as skin rash, colitis with diarrhoea, hepatitis, pneumonitis or thyroid and other hormone disorders, sometimes months after starting or even after stopping; most are manageable when recognised early and treated with steroids or a treatment pause.
These depend on the drug and may include skin rash, diarrhoea, raised liver enzymes, high blood pressure, fatigue, mouth sores or heart rhythm changes; managed with regular tests, supportive medicines and dose adjustments.
When combined with chemotherapy, low blood counts with infection risk, nausea, fatigue and nerve damage (neuropathy) can occur; fever during treatment always needs urgent medical assessment.
Reactions during infusions, and small risks from biopsies or port placement such as bleeding or infection; treatment is given in hospital units equipped to manage them.
Not every tumour responds, and some respond at first and later become resistant; response is checked with scheduled imaging and the plan is revised by the oncologists.
Side effects may appear after returning home; patients leave with a written treatment summary, a list of warning symptoms and a contact plan, and local oncology follow-up should be arranged in advance.
Realistic Expectations & Outcomes
Targeted therapy and immunotherapy can control some cancers well, but they are not suitable for everyone and do not work for every tumour. Molecular profiling may reveal a treatable change, or it may not, and a test result is not the same as an available or approved treatment. Options are decided by the tumour board after reviewing pathology, stage, biomarkers and previous therapy, and the recommendation may be chemotherapy, radiotherapy, surgery or supportive care instead. Response is judged with follow-up imaging, usually after several treatment cycles, and treatment is often long-term, with side effects monitored throughout. Revision & Follow-Up Policy
In oncology, treatment plans are reviewed continuously. The patient coordinator helps share imaging, blood results and symptom reports between the oncology team in Istanbul and the patient's doctors at home, and arranges follow-up consultations whenever the oncologists request them. If the cancer does not respond or side effects become significant, the plan may change through dose adjustments, a different drug or another approach, but only after the tumour board or the treating oncologist has reassessed the case.
